About Private Duty Nursing
Private duty nursing is a specialized service that provides personalized nursing care in the comfort of a patient’s home. This involves sending a registered nurse (RN) or licensed practical nurse (LPN) to work one-on-one with patients with complex medical needs.
Medical requirements
To be approved for these services, patients must have skilled needs such as G-tubes, tracheostomies, ventilators, or be oxygen dependent. Some select insurance will approve patients with seizures.
The process
To get started, please fill out this form. Once you contact us, we will assign a dedicated intake coordinator to guide you through the process. The steps will vary depending on whether the patient is already approved for services or has been discharged. If the patient is approved, we will promptly work to match dedicated nurses and get you started. If the patient is still hospitalized or at home and has not received insurance approval, your coordinator will take care of all the necessary paperwork. This includes obtaining a letter from the doctor and arranging home assessments with the insurance company so you won’t have to worry about anything. We will also coordinate with the hospital or facility’s social worker to arrange for discharge. If services are denied, we are here to help you advocate for what you need. Once approved, we will connect you with compassionate caregivers to ensure you feel comfortable and supported in your home.

  • MLTC- for adult patients, VNS, Senior Health Partners, Fidelis, Amerigroup, United Healthcare, Senior Whole Health, Homefirst, Metroplus on SCA with Centersplan and Riverspring.
  • Commercial insurances in contract: GHI, Cigna, BCBS, The Empire/UHC Plan. We can initiate an SCA with any other plan not listed above.

What is the difference between LPN & RN services?

LPNs, or Licensed Practical Nurses, can perform most duties that RNs can, except for handling certain aspects of a PICC line and verifying the placement of an NG tube.

What if I need to change my nurse?

If you have any concerns about a nurse assigned, please let your coordinator know, and we will find someone who is a better fit.

Who creates the care plan?

An expert clinical case manager is assigned to each case. They create the care plan and then guide the cases throughout.

Do you do services for pediatrics or adults?

We service both pediatric and adult patients, and we have nurses specializing in each.
